FREEZING YOUR MEMBERSHIP 

Your membership can be put on hold for a maximum of four (4) weeks per year. Please email lauren@day56.com.au with your full name and the dates you wish to put your membership on hold. Please let us know the dates you want to freeze your membership for at least 2 weeks prior to you going away. Failing to do so could result in being charged while you're away due to the timing of your direct debit.

CANCELLING OR CHANGING YOUR MEMBERSHIP 

Your membership indicates the minimum length of your memberships. All membership purchased at DAY56 is a minimum of twelve (12) weeks. Once you have completed your first initial 12 weeks, your payments will continue to be debited fortnightly unless a cancellation is requested on your behalf. To cancel or change your membership after the agreed membership term, please drop us an email at lauren@day56.com.au requesting the cancellation of your membership. Cancellation of your membership requires a two (2) week notice period from the day you confirm your membership cancellation.

MISSED PAYMENTS 

If you need to update payment details, please email lauren@day56.com.au so she can assist you in making those changes. This is extremely important as there is an Ezidebit fee of $14.80 for any missed payments. Unfortunately, this is out of our control due to Ezidebit's terms and conditions.

SESSION CANCELATIONS
If at any time there are changes to your health, including disease, medical condition, injury or illness, please inform your Coach before commencing your next session.

If any of the exercises prescribed to you cause you pain or discomfort, please stop immediately and inform your Coach.

All sessions will start and finish at the allocated times. No concession will be made for late arrivals unless otherwise noted, as this can have a significant impact on other member's session times.

Session cancellations for 1 on 1 Personal Training require Twenty-Four (24) hours’ notice. Any sessions cancelled within 24 hours will, unfortunately, have to forfeit the session.

Session cancellations for Group Training require Twelve (12) hours’ notice. There are members on a waitlist in peak class times, so if you do not show up or cancel at the last minute, other members miss out. Mind body is a system where members will automatically go on a waitlist if the class is fully booked, if you cannot make the class, simply cancel out and a notification will go to the first member on the waitlist. At DAY56, we are all about teamwork and building a strong, healthy community, so please give ample notice for any cancellations; this way, no one misses out on getting their session in.

DAY56 does not provide any medical diagnosis or advice regarding an individual’s physical, mental or medical condition. Any and all nutritional information provided to you, is given as guidelines only, it is the sole responsibility of you the client/individual to provide complete and accurate information so that the appropriate guidance can be given. The nutritional guidelines given to me by DAY56 are guidelines only, and I am legally responsible for the food and supplementation I consume.

MEDIA 

I hereby authorise any images, video footage or the like taken of myself, in whole or in part, individually or in conjunction with other images, video footage or the like, to be displayed on the DAY56 Website and other official social media channels, and to be used for media purposes including promotional presentations and marketing campaigns. I waive rights to privacy and compensation, which I may have in connection with such use of my name and likeness, including rights to be written copy that may be created in connection with video production, editing and promotion therewith

LIABILITY 

In consideration of my use of the exercise equipment and facilities provided by DAY56, I expressly agree and contract, on behalf of myself, my heirs, executors, administrators, successors and assigns, that DAY56 and its insurers, employees, officers, directors, and associates, shall not be liable for any damages arising from personal injuries (including death) sustained by me on, about or around the premises, or as a result of the use of the equipment or facilities, regardless of whether such injuries result, in whole or in part, from the negligence of DAY56. By the execution of this agreement, I accept and assume full responsibility for any and all injuries, damages (both economic and non-economic), and losses of any type that may occur to me or my guest, and I hereby fully and forever release and discharge DAY56, its insurers, employees, officers, directors, and associates, from any and all claims, demands, damages, rights of action, or causes of action, present or future, whether the same be known or unknown, anticipated, or unanticipated, resulting from or arising out of the use of said equipment and facilities. I expressly agree to indemnify and hold DAY56 harmless against any and all claims, demands, damages, rights of action, or causes of action of any person or entity that may arise from injuries or damages sustained by me. I agree to be solely responsible for the safety and well-being of myself. I agree to comply with all rules imposed by DAY56 regarding the use of the facilities and equipment. I agree to conduct myself in a controlled and reasonable manner at all times and to refrain from using any equipment in a manner inconsistent with its intended design and purpose. I understand and acknowledge that the use of exercise equipment involves the risk of serious injury, including permanent disability and death. I understand and agree that DAY56 is not responsible for property that is lost, stolen, or damaged while in, on, or about the premises.

DECLARATION 

I am over 18 years of age. (If not over 18 years of age, parent or guardian must sign). I certify that I have fully read and understand the terms of this Membership Agreement and will comply with the contents herein.
